Clinical Nurse Specialist - Community Gerontology Service at Bay of Plenty District Health Board (Tauranga/Whakatane), Bay of Plenty

We have an exciting opportunity for an experienced Senior Nurse to join our team as a Clinical Nurse Specialist. This is a permanent, fulltime position at Tauranga Hospital.
This role has been established as part of the Community Gerontology Service and is responsible for advanced clinical assessment and the management of older adults across inpatient and community settings. The role will assist in minimising hospitalisation through receiving and responding to referrals and facilitating the transition from hospital back into the community through coordinating care delivery and discharge planning.
This role will work with people to assist them in making informed choices around care, provide clinical teams with advice and guidance on treatment of symptoms and management of care - and support transitions in care between hospital and the community.
