He kōrero mō te tūranga - About the Role

The role focuses on working with clients and their families/whānau living with chronic health conditions who have a wide range of high and complex needs. You will be working with clients ranging in age from 18 to 64 years of age.

This is a permanent full-time role working 40hours per week located in Whakatāne.  

The ability to ‘think outside the box' and develop creative solutions to meet complex needs is essential.  

Facilitating holistic assessments to identify clients needs and mitigate risks, and then to coordinate the services are all responsibilities of this role.

The assessment tool used is the online interRAI Home Care tool which required proficiency in Information Systems.

The Case Manager role includes establishing and developing networks with a wide range of individuals and organisations involved in supporting the client ad their family/whānau with regular reviews to ascertain the effectiveness of services provided.
