About the Role
Are you a confident, experienced Registered Nurse with a strong passion for clinical assessment?
We have an exciting opportunity to be involved in a pilot programme designed to support patients in our community to access timely, appropriate care within General Medicine planned care services.
We are seeking a skilled nurse with advanced clinical assessment expertise to work in partnership with General Medicine Physicians to screen and assess General Medicine referrals. This role plays a key part in ensuring referrals contain the appropriate clinical information to enable effective triage and prioritisation of patient care.
The position may involve ordering and following up additional investigations in line with agreed clinical pathways. Strong relationship‑building skills with primary care providers, along with the ability to identify, seek, and interpret further clinical information, are essential.
This is a unique opportunity to work autonomously while remaining an integral member of a supportive, collaborative multidisciplinary team, contributing to improved patient outcomes and service innovation.
About the Team/Service/ Location
Our General Medicine and planned care teams work collaboratively to provide patient and whānau-centred care to the people of our community. Reporting will be to the Clinical Nurse Manager – Outpatients.

About you
To be successful in this role, you will:
- Be a registered nurse with a minimum of five (5) years post-graduate experience
- Be able to demonstrate advanced knowledge in general medicine nursing
- Have strong clinical judgement and advanced assessment skills
- Have excellent communication and teamwork capabilities
- Have ACLS Certification level 6 - desirable
