Our Organisation
Te Pae Hauora o Ruahine o Tararua MidCentral (MidCentral) is part of Te Whatu Ora, Health New Zealand and provides healthcare services to over 180,000 people across the Manawatū, Horowhenua and Tararua regions. With a large hospital campus in Palmerston North, it also provides key health care and support services to neighbouring communities.
The Opportunity
Supportlinks is the Needs Assessment and Service Coordination (NASC) service in the Te Uru Whakamauora (Healthy Ageing and Rehabilitation) directorate which provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral region for people with acute/short term needs, long term conditions, and older people and Kaumātua.
Supportlinks works primarily with a community focus however also has a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged.
An exciting opportunity has arisen for a person to provide operational leadership and day to day management of the Supportlinks service. This permanent full-time position will be based in Palmerston North.
Applicants will need to have:
- Professional health qualification with assessment as part of scope of practice i.e. Occupational Therapist, Physiotherapist, Speech Language Therapist, Social Worker, Registered Nurse
- Holds current Annual Practising Certificate
- Clinical experience in disability settings
- Functional knowledge and provision of interRAI assessment
- Leadership skills and experience
- Knowledge and understanding of the New Zealand Health System as pertains to Kaumātua/ older people or people with long term chronic health conditions
- Current New Zealand Drivers Licence
