About the Service

Supportlinks is the  Needs Assessment and Service Coordination (NASC) service which

provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district for people with acute/short term needs, people who have long term health conditions, and older people/kaumātua with age related health conditions.

Supportlinks work primarily with a community focus, however, also have a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged.

About the Role

Supportlinks currently has a permanent full time 1 FTE position which can be based in either our Palmerston North or Levin office.  We are looking for a person who is enthusiastic, energetic, resilient, and has a “can do” attitude, to join our supportive team and would welcome applications from candidates who hold an annual practicing certificate in any of the following disciplines: Nursing, Occupational Therapy, Physiotherapy or Social Work

  • an ability to work within a changing health environment
  • a mana enhancing and restorative approach within your practice
  • knowledge and working relationships with iwi/Māori providers in the district
  • knowledge of community resources and networks
  • a strong desire to help clients and whānau achieve their goals
  • an ability to communicate with people from a wide variety of backgrounds and cultures
  • a willingness to adapt and flex and trial new ways of working
  • effective written, oral and interpersonal skills
  • strong organisational and time management skills
  • skills to use a laptop and learn new assessment software
  • a full, current, driver's licence
