About the Role

Join us as a Pharmacist Liaison and be the vital link between hospital and community care. You'll review medications, reduce errors, and optimise therapy for patients and aged care residents—especially focusing on safer prescribing and deprescribing. Make a real impact on patient safety, outcomes, and quality of care every day.

About you

You're a NZ-registered pharmacist who's passionate about safer medicines and better patient outcomes. Collaborative, organised, and proactive, you thrive on reviewing medications, resolving issues, and deprescribing where it counts. You bring energy, leadership, and a genuine drive to make a real difference for patients and aged care residents.
