About the Role

As a Kaituitui Whānau Ora Tapuhi (Registered Nurse), you will work primarily in the community to support whānau Māori with complex health needs.

You will play a key role in preventing avoidable hospital admissions, reducing readmissions, and improving discharge outcomes by coordinating care across primary, secondary, and community services.

Working alongside iwi, Māori health providers, and wider health services, you will take a proactive approach — shifting care upstream from reactive hospital care to early intervention and prevention.

This role contributes directly to:

• Improving equitable health outcomes for whānau Māori
• Strengthening discharge planning and continuity of care
• Reducing pressure on hospital services
• Supporting delivery of Pae Ora (Healthy Futures) outcomes

About you

We are looking for a motivated and culturally responsive Registered Nurse who can work autonomously and collaboratively in a community setting.

You will have:

• Current NZ Registered Nurse APC (without conditions)
• Full NZ Driver's Licence
• Experience working in community or complex care settings
• Strong ability to build relationships with whānau and community providers
• Commitment to Te Tiriti o Waitangi and improving Māori health outcomes
• Confidence working independently and across services

Pae Ora Paiaka Whaiora sits within the Māori Health Directorate and focuses on achieving better health outcomes for Māori through integrated, culturally responsive care.

You will be part of a collaborative multidisciplinary team working closely with iwi partners, community providers, and hospital services to deliver whānau-centred care.
