Integrated Services Case Manager - Taranaki

 

Integrated Services Case Managers (ISCMs) assess the complex needs of families and individuals and then links them to the most appropriate support services.

This critical role is responsible for providing an intensive case management service to our clients, supporting them to identify their needs and to overcome a range of challenges to achieve sustainable education, training, employment and housing outcomes. 


You will work closely with key agencies to build collaborative plans at a holistic level.  

This challenging yet rewarding position is best suited to those who can demonstrate a client-focused practice, have highly developed motivational interviewing skills and can deliver an innovative approach to working with others. 

If you are already working in a social services environment or are familiar with case planning and management, this will be an advantage.  

You will also have a high level of personal integrity, take initiative, work well under pressure, and be able to self-manage and work autonomously. 

To support our clients to achieve their goals, you must be able to work closely with your key networks within MSD and in the education, training, housing and employment sectors, in addition to other key community partners in the Taranaki Region. 

You are naturally collaborative, an innovative thinker, and you'll be energised by working with our diverse range of stakeholders to make a real difference in our clients' lives.


To be considered for this role you will have:

•    a genuine passion and commitment to working closely with clients to help overcome complex  barriers and achieve positive outcomes

•    highly effective communication skills (both written and verbal)

•    excellent rapport and relationship building skills

•    cultural sensitivity and responsiveness

•    established community networks and knowledge of supports and services

•    an ability to influence and motivate, sees the potential in others

•    empathy, resilience and tenacity

•    a current full driver's licence

•    face to face client experience

•    a relevant tertiary qualification is desirable, however not a requirement

 
If you have proven experience delivering results in an outcome-based environment where a focus on service and support is paramount, we welcome your application.

Salary range: $71,231.00 - $87,060.00
