Advanced Care Planning Coordinator

Strategy, Funding and Public Health

24 hours per week


Would you like to take a lead role in driving the uptake of Advanced Care Planning across the Waikato District?

We are currently looking for a dynamic, self-motivated influencer who is passionate about Advanced Care Planning as a means of helping people to take control of their own long-term health outcomes.

The successful candidate will work across primary, community and secondary care settings to increase the delivery of ACP support to patients and uptake into the programme. The position will have the opportunity to participate at local, regional and national strategic planning forums to increase the awareness and participation in ACP activity generally.

The successful candidate should already have a good knowledge of ACP and possibly some experience in the area.  You should have a clinical background, be able to demonstrate experience in leading change, and be adept at developing enthusiasm and commitment to new approaches across broad stakeholder groups. Confidence in driving performance outcomes and the ability to engage in Māori settings is essential.

Initially this is being established as a 0.6 FTE 12 month role.  It will be reviewed after this time on the basis of its success.  The DHB is flexible around the hours worked over a 24 hour work week.
