Right now, Northland DHB is looking for an enthusiastic and committed Registered Nurse to join the team as a Nurse Coordinator. This role will see you responsible for the coordination of patients with complex discharge planning into District Nursing. You will also support the District Nursing team by providing clinical expertise and direction to your team to achieve the best outcome for patients.

To be successful for this role, you will need:

• A minimum of 3 years in a relevant specialty area
• Experience in Community Healthcare (experience in District Nursing or Primary Health Nursing is desireable)
• Well-developed communication skills: with patients, their families and the inter-disciplinary/medical teams
• Confidence with computer based systems for patient management
• Positive, adaptable, flexible and motivated
