Discharge Planner 

Salary Band – $80,000 - $112,000 p.a prorata + Kiwisaver 

 

  • Southland Hospital
  • 0.4 FTE (32 hours per fortnight)
  • Fixed-term until September 2026

About the role

Assisting with the temporary increase in beds for the winter period - we are seeking a health care professional can be either Allied or Nursing.

The purpose of the Discharge planner is to:

  • Lead the collaboration of the inter-professional team 
  • Ensure all patients have a clear disharge plan to facilitate safe discharge of patients, from inpatient services.
  • Ensure all complex patients are reviewed within 12- 24 hours of admission to facilitate care coordination and discharge planning needs.
  • Reduce ED admissions through identification of appropriate community/outpatient pathways 
  • Collaborate with all health professionals as well as the wider multi-disciplinary team

  • Must have a current registration and APC as a registered health professional, and the right to work in New Zealand.

Salary Range
This role is covered by the terms and conditions of the PSA Allied Public Health & Technical CEA and the salary will be in accordance with the Core Pay Scale, between $80,594 and $112,496 p.a dependant on relevant experience.

This role is covered by the terms and conditions of the expired NZNO CEA and the salary will be in accordance with the RN Pay Scale, between $81,683 and $106,739 p.a dependant on relevant experience.
