• Nursing salary band – $81,683 - $106,739 p.a pro rata

About the Role

Our Home Team is available to provide supported discharge of patients working across a range of settings including the Emergency Department, in-patient areas and community,  as well as hospital avoidance.  They are focused on delivering one car up the driveway, one assessment and one plan of care.

This is a fantastic opportunity for a Registered Nurse interested in working as part of an Inter-professional team.

Tasks for this role include but are not limited to:

• Support the early discharge of appropriate patients from Southland Hospital
• Monitor their recovery, ensure everyone is able to maintain their independence in their own home
• Working in a comprehensive, integrated service that are responsive to patient and service needs
• This role covers a Monday to Sunday service between the hours of 8 am to 5 pm and also includes public holiday work.

About you

This role requires a nurse who is adaptive to change, resourceful, flexible to service needs and future development and a genuine passion for working in the community.

Our ideal candidate will have:

• A current APC and be a Registered Nurse with Nursing Council of New Zealand
• At least two years post registration nursing experience in New Zealand
• A clean full New Zealand Drivers licence
• Comprehensive clinical assessment and problem-solving skills.
• Ability to work autonomously and collaboratively within the inter-professional team.
• Ability to direct and delegate to Allied Health Assistants.
• Excellent communication skills to liaise with health professionals, both within the in-patient services and in the community setting.
• Recent experience of working with the older adults in acute, rehabilitation or community settings

To be successful in the role you will have a strong commitment to your ongoing professional development, and it is expected that you will have / or be committed to working towards a post-graduate pathway. Competence in assessment of complex patient needs is desired.
