Addictions Case Manager at Health New Zealand - Te Whatu Ora Nelson Marlborough, Nelson Hospital

About the Role

We are seeking an enthusiastic and experienced Addictions Case Manager, who is either an allied health professional, or registered nurses, with experience, preferably within Addictions or mental health setting.

As a Case Manager, you will work collaboratively with clients, their whānau, and support networks to provide comprehensive assessment, care coordination, treatment planning, and recovery-focused interventions. You will work alongside individuals experiencing substance use challenges, often in combination with mental health concerns, supporting them through withdrawal and recovery. This includes facilitating medicated withdrawal where appropriate, promoting freedom from addiction, enhancing wellbeing, and empowering people to live fulfilling lives.

The role offers variety and autonomy, with opportunities to work across community settings and alongside a wide range of health and social service providers.

About the Team/Service/ Location

Our service provides specialist community-based addiction and mental health support for individuals experiencing substance use challenges and co-existing mental health concerns. We work from a recovery-oriented framework that promotes resilience, empowerment, and partnership with clients and their whānau. Our focus is on supporting people to achieve meaningful outcomes, improve their wellbeing, and live fulfilling lives within their communities.

You will be joining a supportive and experienced multidisciplinary team comprising clinicians from a range of professional backgrounds who are committed to delivering high-quality addiction and mental health services.

About you

To be successful in this role, you will have:

  • A current New Zealand Annual Practising Certificate within your relevant professional discipline, preferably with an addiction focus, or be working towards an addiction-specific qualification
  • The ability to provide high-quality assessment, intervention, and care coordination within a recovery-focused framework.
  • A commitment to delivering responsive, holistic, person-centred care that supports individuals and their whānau to achieve their recovery goals.
  • An understanding of contemporary addiction and mental health practice, including harm minimisation and recovery principles.
  • The ability to work effectively both independently and as part of a multidisciplinary team.
  • A commitment to cultural safety, equity, and improving health outcomes for diverse populations.
  • Excellent organisational skills, sound clinical judgement, and the ability to manage a varied caseload.
