About the Role

The Needs Assessor/Service Coordinator role focuses on assessing the needs, strengths, goals and resources of people aged 65 years and over who are living with long term health and/or disability related conditions.

You will complete comprehensive InterRAI Home Care assessments in partnership with clients, their families and whānau, identifying the support required to help people remain safe, independent and active in their communities.

Working across both community and inpatient hospital settings, you will use your clinical expertise and sound judgement to identify needs and risks, coordinate appropriate services and connect clients with government and non government funded support.

This is a varied and rewarding role where you will build relationships with clients, whānau, health professionals and community providers. Travel throughout the Bay of Plenty is an important part of the role.

Training in the InterRAI system will be provided, however, a strong level of digital confidence, adaptability and a willingness to learn are essential.

We are looking to appoint:

One position at 0.9 FTE (36 hours per week)

One position at 1.0 FTE (40 hours per week)

About the Team/Service/ Location

Support Net is part of the Regional Community team based in Tauranga. The Health of Older People team works in collaboration with kaumātua, whānau, community providers and other health professionals to ensure safe, effective and person-centred support is in place.

You will be part of a team committed to supporting older people to achieve their goals, maintain their independence and remain connected to their communities.

About you

What you will bring:

• It is essential that you are a Registered Health Professional with a current Annual Practising Certificate
• A current full NZ drivers licence and willingness to travel for client visits
• Strong communication and interpersonal skills
• Competent computer skills and confidence using clinical information systems
• The ability to work independently, manage competing priorities and use sound judgement
• A collaborative approach and the ability to build effective relationships across the health and disability sector
• A commitment to professional, ethical and culturally safe practice
• A flexible and adaptable approach, with a willingness to learn and embrace change

Preferred skills but not essential

• Experience working with older adults and their whānau
• Knowledge of health and disability services
• Experience working in a community setting
