We have a great opportunity for a senior registered nurse with strong leadership and advanced nursing skills to join our team as a Clinical Nurse Specialist.The HBP CNS role is an advanced nursing practice role that encompasses responsibility and coordination of nursing care to this patient group across health care settings both locally and regionally.
This is a key role within the Surgical Services Team and has a strong focus in General Surgery. Your primary role will be to provide care co-ordination –maintain a database and tracking system to ensure patients receive timely interventions. You will act as a point of contact for patients and the multi-disciplinary team-provide you contact details, check and respond to phone calls, emails and messages related to patients and whanau accessing the service. You will be a Support for patients, families, nursing staff and medical staff on the wards-refer to multi-disciplinary team as required; organise family meetings as required; assist with discharge planning and communicate plans to patient, whanau, team and local DHBs
To ensure success in the role you will need to have an extensive surgical nursing background and be practicing at a senior level preferably with a postgraduate certificate or diploma in speciality nursing practice.
You will be joining a supportive senior nursing team who are committed to effective/evidence based nursing practice. You will also have the opportunity to work autonomously.

This is Monday to Friday opportunity working 40 hours per week (1 FTE).
