Fixed Term Position – Full-Time and Part-Time Roles

 

 

We are looking for experienced nurses to work in the Community Health Service within a centralised Hospital in the Home team.  In this role you will provide acute community based nursing for patients to support admission avoidance and early support discharge pathways. 

 

The CNS role will work as a member of an interdisciplinary Hospital in the Home team which includes SMO, Nurse Practitioners, District Nurses, Allied Health and HCA staff working across the Counties Manukau Health district. 

 
 
 

The Hospital in the Home team will provide a mixture of remote monitoring and face to face patient assessments/intervention with rosterred shifts across 8am-8pm, Monday - Sunday.  You will provide support and intervention across both COVID and standard acute care pathways and at times support routine District Nursing for the Community Health Teams.

 
 
 

The Clinical Nurse Specialist roles will provide expert, evidence-based care to optimise patient outcomes and ensure the services delivers high quality care. CNS clinicians will be effective role models demonstrating high levels of nursing competence and knowledge sharing, provide leadership, guidance and support within the team and will actively establish relationships with other stakeholders

 
 
 

You will be required to initiate and develop innovative approaches to problem solving while utilising skills and input from other disciplines and team members to co-ordinate patient care.

 
 
 

You will be empowered to make decisions or recommendations relating to patient management, while demonstrating a high level of communication and professionalism.

 
 
 

Essential requirements:

 
  • Registered with the NZ Nursing Council
  • Completed Post Graduate Diploma
  • Functioning at an RN Expert level of PDRP
  • A minimum of 5 years recent clinical experience preferably in primary or community nursing care

  • Knowledge and experience in long-term conditions care coordination and case management

  • Full Drivers Licence
  • Functioning at proficient level on PDRP
