Our Needs
Assessment team works with people with age related or chronic conditions.
Together we help identify their individual challenges, goals, and steps needed
to achieve these goals, through assessment and service coordination.
You will work with our team based in the Blenheim Health Hub and you will have close working relationships with Wairau hospital, other health agencies, community services and Iwi/Maori Health providers. We will value your commitment and experience in working with people and their families to ensure that their needs are at the centre of their care. This is a permanent, full time position working 80 hours a fortnight (Monday - Friday between 8am and 5pm).

You will be a motivated and enthusiastic individual, ideally with experience working with individuals with health related disabilities. You will also be a registered health professional and hold a full driver-s licence.
For the health and safety of staff and patients all NMH staff are required to be vaccinated in accordance with the COVID-19 Public Health Response (Vaccinations) Order 2021.
