Ward 5 is one of five Assessment Treatment & Rehabilitation wards at Middlemore Hospital, providing patient focused care in Health of Older People. In addition, Ward 5’s ACE (Acute Care of the Elderly) model of care focuses on acute geriatric evaluation, assessment and treatment with a multidisciplinary focus. The service provides care for patients aged 80 years old and older with acute medical conditions. It has a particular focus on managing frailty, complex needs, multiple co-morbidities and patients who will likely require a rehabilitation period following their acute care episode. The goal is to improve the care and management of these patients by providing a multidisciplinary geriatric unit that co-ordinates acute care with rehabilitation services.
