Kōrero mō te tūranga - About the role
We are looking for a full time Long Term Conditions/Respiratory Clinical Nurse Specialist (CNS) who will contribute to the development of nursing practice in the community setting and promoting self-management strategies for clients with Long term conditions, using self-management skills such as, problem solving, decision making, partnership, and action planning and self-tailoring.
The role will suit a registered nurse with advanced clinical knowledge and expertise in Long Term Conditions/Respiratory nursing practice. The role provides clinical leadership and develop the of the District Nursing teams.
The position supports the acute flow of patients discharging from the HSS into the C&C region within the community setting and supports GP practice & primary care clients. The Long Term Conditions/Respiratory CNS position supports the District Nurses in the community setting with clients receiving home oxygen & has clinical overview of the Community Health Service (CHS) Pulmonary Rehabilitation programme.
If you hold (or are working towards) a Master-s level qualification this could be the position for you. We would like the Long Term Conditions/Respiratory CNS to work towards nurse prescribing.
The position is full time (1.0 FTE), Monday to Friday with some flexibility to work afternoons and weekends if the service requires.
Mō tō mātou rōpū - About our team
District nursing service support the discharge of patients from hospital health service (HSS) into the Wellington region, and supports GP practice community patients. District Nurses work in the community setting with patients receiving home intervention such as intra venous therapy, palliative care, wound care, compression bandaging, Doppler assessment, medication management, stoma care, incontinence care and respiratory care, home oxygen therapy. The district nurses work with Community Health, Health Care Assistants (HCA-s) team, assessing patients who require home personal cares and then handing over the patient care to the HCA-s.
Moū - About you
This position would suit an experienced RN with driver licence who has background in community, primary health care or medical/respiratory nursing, who would like to work in an autonomous role. The specialist Respiratory Long Term Conditions role that is based in the secondary health sector.
