Kōrero mō te tūranga ' About the role
This is a full time, permanent position working with Renal patients aged 16 years and over across Wellington Hospital and Wellington, Kenepuru and Hutt dialysis units as well as in the community. The successful applicant will complete psycho social assessments with patients and their whānau and work in partnership with their multi-disciplinary colleagues, Renal Clinical Nurse Specialists and Renal doctors.
In this role, the social worker provides clinical care to people and their whānau to help them manage the psychological and social impact of kidney failure in their lives, as well as assisting patients to navigate complex health and social care systems. The Social Worker will utilise their therapeutic skills to support patients and their whānau to process their emotions and reactions through their health journey.
The role provides cover into other areas of the hospital as needed and social work on Saturday's across inpatient services on a roster basis, approximately 3 -4 times per year receiving penal pay rates.

We are a team of 22 social workers who provide a service to patients and their whānau across inpatient, outpatient and community settings. We complete psycho social assessments across the age range and provide holistic social work interventions. Come and join our friendly experienced team in a supportive workplace environment.

As the successful candidate you will have a passion for working in this area and be highly motivated to support patients and whānau navigate their health journey. You will be able to work collaboratively and effectively as part of a multi-disciplinary team and with external agencies. Working with a diverse group of patients with significant health needs the post holder will require excellent documentation, communication and time management skills. This is a great opportunity for someone who is keen to work in this clinical area.

The successful applicant will have the following skills and attributes:
- NZ Registered Social Worker with current annual practicing certificate
- Minimum of 3 years social work practice, although those with less experience will be considered
- The ability to complete psychosocial needs assessment
- Navigating and working within the legal requirements of health care and social work
- A clear understanding of the health needs of Māori and Pacific Island communities
- Therapeutic social work counselling experience (desirable)
- Previous work in a health setting and a good understanding of health social work (desirable)
