Consider joining our helpful Community Service Hub to undertake this important role. You will assess clients and create a plan of care for them. Thereafter, you will coordinate client care and liaise with external providers who will go on to provide treatment to clients based on your plan of care. The regular client profile is aged over 65 years of age with aged related disabilities or chronic health conditions, while the other client base you will be working to support are those who require service coordination due to a Mental Health need.
All assessments will be carried out using the appropriate assessment tools for the service. You will be provided training if needed. You will manage the client and family expectations, providing services within established funding guidelines and timeframes.
You will work autonomously for the most part and also in rotational roles alongside other members of the NASC team. You can be working in triage, hospital liaison and out in the community. Every role is different, giving you the chance to learn and grow as a healthcare practitioner.
Allied Health Professionals and Registered Nurses will benefit being under the Multi-employment Collective Agreement (MECA) and have the opportunity to progress through either Merit Steps or Professional Development Recognition Programmes (PDRP).
