Kōrero mō te tūranga ' About the role
We have an exciting opportunity for an experienced Physiotherapist to join our Advancing Wellness at Home Initiative (AWHI) team. AWHI is CCDHB's Early Supported Discharge service for people who have been discharged from both Wellington and Kenepuru Hospitals. There are currently two AWHI streams which include Medical/Health of the Older Person's and Stroke. The medical stream will soon be expanding to include work with patients on the ACC Non-Acute Rehabilitation contract. Physiotherapist's in AWHI practice in an interprofessional model to deliver reablement and promote self-management of health for the people we work with in their own home. The role also involves working in the hospital to identify patients who will benefit from the service. This role is based in the North Team at Kenepuru Hospital and will work across both the Stroke and Medical streams.

You will be joining a friendly, fun and enthusiastic team from a mix of professional groups. We offer on-going professional development opportunities including inter-professional support and training with the team of approximately 24 FTE around you. You will also have access to professional support and profession specific training though the relevant professional lead and clinical supervision. Cultural professional supervision is available from Māori and Pacific Health teams. The team is continuing to grow and there will be opportunties to innovate and develop the service. The team is split over 2 bases; Ewart at Wellington Hospital site for the southern base and Kenepuru Hospital for the northern base. The 2 parts of the team cover all of CCDHB's catchment area between them, Seatoun up to Peka Peka but each team member has an identified base.
Moū ' About you
We are looking for a Physiotherapist who has current registration/APC and has recent clinical experience working with people under general medicine/ health of the older person and stroke rehabilitation specialties. The ideal candidate will have a minimum of 3 years' experience and will hold a current NZ driver's license. You will be able to demonstrate ability to build rapport with patient and whānau from all backgrounds and have a clear understanding of the health needs of our Māori and Pacific Island communities. You will have a flexible approach to working in a high turnover, fast paced environment and have an understanding of interprofessional practice. If you are passionate about being the best you can be, thrive on working in a team setting and are ready to expand your practice further then we'd love to hear from you.
