The Pacific Health team of the Te Whatu Ora Health New Zealand Te Matau a Māui Hawke’s Bay are looking for a Pacific Nurse Case Manager to support the delivery of our fanau ola service for Pacific people with complex clinical and cultural needs. You will ensure they have an understanding of their health and wellbeing, and are supported in their treatment. You will also have opportunities to support and input into community led initiatives as part of the Pacific community health and wellbeing village and to share with other primary and secondary care clinical teams, services and our public health team. Your work will contribute towards supporting the health and wellbeing of our Pacific community through these difficult COVID times.
What you will bring:
You will be confident working across diverse environments and clinical teams and have good interpersonal and communication skills. You will also bring:
- A qualification in nursing
- Commitment and understanding of Pacific cultural values, processes and principles
- Basic proficiency in one or more Pacific language’s
- A tertiary qualification in a relevant discipline
