The successful applicant will be working predominantly within the Eastern locality, as part of the wider IDT to provide excellent delivery of care to our clinically and culturally diverse population within the community setting.
Clinical delivery will be focused predominantly within the Hospital-in-the-Home and Reablement workstreams. This role will focus on the acute-care-in-the-community and restorative specialist areas and manage people with high clinical complexity to maintain their quality of life in their own homes and communities.
Clinical work is within the Pukekohe Hospital inpatient service – a rehab focused role to work alongside GP’s, Geriatrician, Nurses, Pharmacists and the other Allied Health professional groups.
Community Central is a triage service comprising of an inter-disciplinary team who work together to apply a rigorous assessment process at the point of referral. This ensures that any immediate and/or additional needs of the patient are clearly identified and recorded. The referral is then forwarded to the most appropriate health care professional. Community Central is based at 15 Lambie Drive in Papatoetoe. It is a 7 day a week service.
- Registration or eligibility to be registered with the NZ Physiotherapy Board with current Annual Practising Certificate
