Bring your exceptional clinical skills to the Nurse Case Manager role focusing on health management of our Pasifika patients and families supporting their journey to access services to address their health needs!
Reporting to the Fanau Ola Nurse Lead you will be will be responsible for providing clinical care for the management of Pasifika patients who meet the criteria, who are referred to the Pacific Health Development Team for assessment and support within the Hospital or community.
You will work actively in partnership with the medical and nursing staff across the care continuum to ensure safe, patient and family centred care in line with the vision and values of Counties Manukau Health, Te Whatu Ora.
The aim is to ensure Pasifika patients and families who require healthcare are better managed, supporting them to receive to timely access to their healthcare. The Nurse Case Manager – Pacific Health Development will be responsible to support individual patient pathways across the primary, secondary and tertiary care interface, establishing effective working relationships with members of the multidisciplinary team, patients, their families and care givers.
To be successful in this role you will be Registered Nurse, with a current NZ practising certificate who has a passion to work in Pacific Health, with a focus on making a positive difference for Pasifika people and communities. This role will see you, working in the Fanau Ola, Pacific Health Development team consisting of a team of Registered Nurses and Social Worker clinicians actively engaged in supporting them in their health journey as well as the development of programmes within the hospital services and community that support better health outcomes for Pasifika. As such it is preferred that you are fluent in a Pasifika language, but not essential. The role also requires you to have experienced computer skills as you will be working with computer programmes and systems.
