Kōrero mō te tūranga - About the role

An exciting opportunity exists for a Mātanga Tapuhi Nurse Practitioner (NP) to join the Older Persons Mental Health (OPMH) Service in the Hutt Valley, a wonderful suburban location just 15 minutes from the beautiful Wellington city. 

This multidisciplinary team provides assessment and treatment for older adults with late onset moderate to severe mental health presentations, those with chronic mental illness with physical frailty, and those with behavioural and psychological symptoms related to dementia. This position is ideal for an experienced Mātanga Tapuhi NP with specialist mental health expertise and skill working with patients whose health is impacted by frailty and age related conditions. 

The Mātanga Tapuhi NP will provide and facilitate comprehensive clinical care and advanced nursing expertise for patients with complex needs. This includes consultation, direct care and treatment as well as providing support through clinical advice and education to relevant staff.  The role includes opportunities to lead and contribute to service improvement initiatives, working with a team committed to high quality specialist care.  Team work is essential in this role as it ensures older patients with mental health presentations and their whānau have access to timely responsive specialist input.

Mō tō mātou rōpū - About our team

You will be joining a cohesive and experienced specialist team who are passionate about improving health outcomes and promoting quality of life for older adults and their whānau. The OPMH service includes Psychiatrists with a speciality in older age, Social Workers, Occupational Therapists, a Psychologist, Community Psychiatric Nurses and the advanced practice Mātanga Tapuhi NP position. The team is based at Hutt Hospital and see patients primarily in the community (patient - s homes and facilities) or in outpatient settings.  We work closely with our Gerontology colleagues and other mental health services across the district. Inpatient consults are in collaboration with the patient - s treatment team. 

Moū - About you

We are looking for an Mātanga Tapuhi NP with:

Expertise in specialist level mental health nursing and relevant older person - s health experience with gerontology knowledge 

An established ability to work autonomously and effectively in collaboration with other health professionals

Mātanga Tapuhi NP registration with the Nursing Council of New Zealand (NCNZ)

The successful applicant in this role will have:

Excellent communication and interpersonal skills and the ability to work effectively with all levels of staff 

A strong patient care focus that is whānau inclusive, underpinning an ability to consistently deliver high quality patient care

A passion for ongoing learning and development and the ability to share, support and develop others.

Proven ability to work collaboratively and flexibly

Comprehensive knowledge of the NZ Health System including equity, professional leadership and emerging issues for the nursing profession
