Planned Care Coordinator at Te Whatu Ora - Health New Zealand Nelson Marlborough, Tasman Community

Ngā mōhiohio e pā ana ki tēnei mahi (About this position)

This district-wide position based in Nelson exists to plan, lead and coordinate the implementation, maintenance and monitoring of strategies to manage district-wide planned care service delivery in accordance with the objectives of Te Whatu Ora / Health NZ. 

This role will involve process and data entry quality improvement work, a high level of data analysis and liaison with key stakeholders.

You will ensure that Planned Care is well co-ordinated across services and the region, there is equity across our district in terms of access and delivery, and that when we commit to patients we deliver against that commitment. 

You will also be responsible for the visibility of all Planned Care activity and performance and will work in partnership with all Service Managers, Clinicians, Administrative staff to take a co-ordinated approach to service planning and delivery. You will become integral in linking with the regional planned care operational teams to ensure alignment.

This is a permanent, full-time position working 80 hours a fortnight. Travel to Blenheim will be required.

  • Having previous experience working in a clinical environment would be an advantage. 
  • Experience in project management and systems development, as well as the ability to think laterally and integrate different levels of perspective are essential to the role.

The successful candidate must be eligible to work in New Zealand or hold a valid NZ work visa at the time of applying.

Te Whatu Ora staff must have completed their primary course of two NZ approved COVID-19 vaccinations.
