Kōrero mō te tūranga - About the role
This is an exciting opportunity to work on our older persons and rehabilitation ward as part of a multi-disciplinary team. This is a full time permanent role. Your work with patients and their whānau will include:
- Assisting when complex social situations impacting on discharge planning
- Care giver stress
- Capacity issues present, or legal protective mechanisms are being considered
- Elder Abuse and Neglect
- Acute adjustment to health
Facilitating MDT and whānau hui - s:

The role requires collaborative working relationships with all Allied Health and medical staff in the hospital and community setting to ensure a seamless transition of patients/clients between acute, rehabilitation and community services. This will take the form of assessment, intervention and advice. You will also provide cover across other inpatient areas as required.
This patient centred role is suitable for an experienced social worker who has worked in health, has a good understanding of disability, patient rights, and the legal frameworks which apply to vulnerable adults.
We are looking for someone who has strong problem solving skills, leadership skills, works well as part of a team, can communicate well with a wider range of people, and has experience working with older people and their whānau.
You will be supported by your social work colleagues and the Professional Leader for social work, along with regular supervision.
Mō tō mātou rōpū - About our team
The inpatient social work team are a friendly, supportive team who provide secondary health social work services to patients/clients within the hospital setting. Services are provided on all of the inpatient wards including Medical Assessment and Planning Unit (MAPU) and Emergency Department (ED).

As the successful candidate you will have a real passion for working in health social work and be highly motivated to improving health outcomes for patients and their whānau. You will be able to work collaboratively and effectively as part of a multi-disciplinary team and with external agencies. The successful applicant will have the following skills and attributes:
- NZ Registered Social Worker with current annual practising certificate
- Minimum of 4 years social work practice (at least two in health)
- The ability to complete psycho social needs assessment
- Strong understanding and experience working with the Protection of Personal and Property Rights Act (1988)
- A clear understanding of the health needs of Māori and Pacific Island communities
