Clinical Assessor (Dementia Specialty) - Community Older Persons Mental Health at Te Whatu Ora - Health New Zealand Waitaha Canterbury, Christchurch

We're looking for passionate Clinical Assessor (Dementia Specialty) to join our Community Older Persons Mental Health team.
- (0.8FTE) Fixed-term position available for a Registered Nurse or Allied Health professional until 4th December 2024;
- Registered Health Professional (Nursing, Social Worker, Physiotherapist, Occupational Therapist) with current APC;
- Competent in the use of the InteRai assessment tool would be beneficial but not essential.

Our Older Persons Mental Health service predominantly provides clinical services to clients living in the community. Based alongside a dedicated multi-professional team, you will be working alongside a range of clinical colleagues and developing services to meet the needs of the community.
Within this role you will be required to rotate between with Older Persons Mental Health Day Clinic, Wards and Outpatients.

Working as a Clinical Assessor is about assessing clients - needs. That could mean supporting them and their families in the community with home supports or advocating a move to Rest Home or Hospital Level of Care facility.
Our service is forward thinking and we - re one of the teams in New Zealand using InterRAI to effectively assess the needs of our patients. We have a restorative model of care enabling the people we work with to remain independent in the community.
The Clinical Assessor is part of the interdisciplinary team and is the key link in the care chain from the ward to community and GP services. Therefore, you - ll understand how important it is to be patient focussed and you - ll hold a who - s who of services for Older People in your head.
- Registration with an Allied Health professional body and a full NZ driving license is a must, as is your enthusiasm and dynamic style.
- There is one (0.8FTE) Fixed-term role available until 4th December 2024 available.
