About the role
As a Kaiāwhina, you will provide essential practical and emotional support to patients as they transition from hospital to home. This role is crucial in ensuring patients feel safe, supported, and empowered in their recovery journey.
Key elements of this role will include:
- Assisting allied health professionals to deliver rehabilitation programmes within people - s homes.
- Sourcing, delivering and set up of equipment to promote health, wellbeing and maintaining independence.
- Plus, other tasks delegated by registered Allied Health professionals.
The working hours are between 7am-7pm Monday to Sunday.

About the Team/Service/Location
An Early Support Discharge (ESD) Service is being established in the Nelson Tasman region, designed to help patients transition smoothly from hospital to home with the right support. The team will work closely with patients, whānau, and healthcare providers to improve recovery outcomes and independence in the community. We are seeking passionate Kaiāwhina to join our dynamic team and make a real difference in people's lives.
About you:
Key Responsibilities:
- Using a rehabilitative approach, assist patients with daily activities such as meal preparation, personal care, and mobility support.
- Provide cultural and emotional support to patients and their whānau.
- Monitor patient well-being and report any concerns to the healthcare team.
- Promote independence and confidence in patients' home environments.
Qualifications
- It - s essential that you have Year 11 - NCEA Level 1 English and Maths (or equivalent) and a current full driver - s licence.
- The NZQA Level 3: New Zealand Certificate in Health and Wellbeing - Health Assistance Strand, if not previously obtained, is to be completed within 2 years of commencement.
Skills & Experience
- Previous health sector experience is essential
- A caring and compassionate nature with strong communication skills.
- Ability to work independently and as part of a team.
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
- Experience in projects / initiatives which give effect to Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.
