About the role
As a Physiotherapist in our Early Support Discharge (ESD) Team, you will play a key role in helping patients regain mobility and independence after hospital discharge. You will work within a multidisciplinary team to deliver person-centred care in home and community settings.
As part of our team, you will work in a collaborative setting, sharing expertise and resources to deliver the best possible outcomes for our patients. You will play a key role in assessing, planning, and delivering individualised treatment programs. You will support a variety of outpatient cases, collaborating closely with multidisciplinary teams to provide holistic and effective support.
The working hours are between 7am-7pm Monday to Sunday.

About the Team/Service/Location
An ESD Service is recently established in the Nelson Tasman region, designed to help patients transition smoothly from hospital to home with the right support. Our team works closely with patients, whānau, and healthcare providers to improve recovery outcomes and independence in the community. We are seeking passionate professionals to join our dynamic team and make a real difference in people's lives.
About you:
Key Responsibilities:
- Assess, plan, and implement rehabilitation programs for patients transitioning from hospital to home.
- Provide hands-on physiotherapy interventions to improve mobility, strength, and function.
- Educate patients and whānau on safe movement techniques and recovery strategies.
- Collaborate with other healthcare professionals to ensure coordinated care.
- Monitor patient progress and adjust treatment plans as needed.
Skills and experience
We are seeking a candidate who has:
- NZ registration in Physiotherapy and a current practising certificate with the Physiotherapy Board of New Zealand.
- Demonstrated rehabilitation experience from either a hospital or community setting is preferred.
- Strong organisational skills and the ability to work independently and as part of a team.
- Excellent communication and interpersonal skills, with a compassionate approach to patient care.
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
- Experience in projects / initiatives which give effect to Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.
