The Role:
We are seeking an experienced Clinical Nurse Specialist (CNS) - Pacific to join our team at Health NZ Counties Manukau. This role focuses on providing expert nursing care and care coordination for Pacific patients within the Critical Care Complex at Middlemore Hospital. You will work closely with the Planned Care – Pacific and Fanau Ola, Pacific Health team, ensuring culturally appropriate and patient-centred healthcare.
Key Responsibilities:
- Provide culturally responsive, expert nursing care to Pacific patients.
- Coordinate care for patients with complex health and social needs.
- Engage and support families (aiga, kaiga, magafaoa, kopu tangata, vuvale, famili) in care planning.
- Facilitate safe transitions from hospital to primary and community healthcare providers.
- Lead case reviews, quality improvement activities, and provide clinical expertise.
- Collaborate with medical, nursing, and allied health teams across the healthcare system.
- Advocate for Pacific health priorities and enhance patient engagement.
About You:
- NZ Registered Nurse with a current Annual Practicing Certificate
- 5+ years of clinical experience in Critical Care in NZ
- Relevant Postgraduate qualification completed or near completion
