- Registered Nurse or Allied Health professional (Social Worker, Physiotherapist, Occupational Therapist) with current APC
- Competent in the use of the InteRai assessment tool would be beneficial but not essential

About the role
Based at Princess Margaret Hospital, our South/West Community Services Team is a large interdisciplinary team working with clients in the community with a goal of maintaining patient independence. We offer assessment, treatment and rehabilitation of older persons.
About You
Working as a Clinical Assessor is about assessing clients - needs. That could mean supporting them and their families in the community with home supports or advocating a move to Rest Home or Hospital Level of Care facility.
Our service is forward thinking and we - re one of the teams in New Zealand using InterRAI to effectively assess the needs of our patients. We have a restorative model of care enabling the people we work with to remain independent in the community.
The Clinical Assessor is part of the interdisciplinary team and is the key link in the care chain from the ward to community and GP services. Therefore, you - ll understand how important it is to be patient focussed and you - ll hold a who - s who of services for Older People in your head.
- Registration with an Allied Health professional body and a full NZ driving license is a must, as is your enthusiasm and dynamic style
