Social Worker - Older Persons Mental Health at Health New Zealand - Te Whatu Ora Te Matau a Māui Hawke's Bay, Hawkes Bay

About the Role

As a Registered Social Worker in the Older Persons Mental Health team, you will deliver safe, evidence - based mental health assessment, intervention and recovery-focused support for older adults experiencing mental health challenges.

You will:

  • Manage a caseload of increasing complexity
  • Provide psychosocial assessment, treatment planning and care coordination
  • Work collaboratively within a multidisciplinary team
  • Support whānau through education, planning and advocacy
  • Contribute to service improvement, supervision of students, and interprofessional teaching
  • This is a role where you can utilise your advanced communication, clinical reasoning and relationship - centred practice to support improved outcomes for tangata whaiora and their whānau.

About the Team/Service/ Location

Our Older Persons Mental Health team provides specialist assessment and treatment for kaumatua with mental health needs across Hawkes Bay.

We are a supportive and collaborative team of Social Workers, Nurses, Psychologists, Psychiatrists, and allied health professionals who work together to deliver high quality, culturally responsive services.

About you

As a Social Worker, you will bring:

  • Bachelor of Social Work (or higher) recognised by NZSWRB
  • New Zealand Social Worker Registration + current APC
  • 5+ years experience in mental health preferred
  • Experience working in community or older persons mental health
  • Strong clinical assessment, communication and engagement skills
  • Ability to work effectively in a multidisciplinary setting
  • A clean full NZ driver licence
  • Understanding of Te Tiriti o Waitangi, equity, Māori health needs and culturally safe practice

Desirable:

  • Membership with ANZASW
  • DAO authorisation (or willingness to train)
  • Counselling skills
  • Experience supervising students
