Care Manager Tasman Mental Health perm 0.8FTE at Health New Zealand - Te Whatu Ora Nelson Marlborough, Tasman Community

  • Care Manager, Registered Nurse, Occupational Therapist or Social Worker
  • Primarily based in Tasman Community, with offices in Richmond or Motueka
  • Permanent role
  • 0.8FTE working 64 hours a fortnight
  • Working between the hours of Monday to Friday, 0800-1700. There is some flexibility to consider within these times regarding working days and hours.

About the Role

As a Care Manager you will be providing a community-based service working with adult consumers with medium to long-term serious mental illness and will work on a Monday to Friday basis.

Our community assessment team provides high quality and comprehensive assessment and treatment for adults and their families living in the Nelson/Tasman region. We value team culture of collaborative practice and expect openness, respect and flexibility across disciplines. You will be also working alongside the wider sector of Adult Mental Health services. Maori Health team Te Waka Hauora, Addiction services, Wahi Oranga (inpatient) and The Community Assessment Team.

About you

You will be a Registered Nurse, Occupational Therapist, or Social Worker with the ability to work effectively both within a multidisciplinary team and autonomously. Liaise with tangata whaiora, whanau and support kaimahi in implementing management/treatment plans. At least two years post registration experience working in mental health, with strong assessment and risk management skills. The ability to engage with primary services and foster a provisional care within discharge planning.

You will be experienced in providing community-centred mental health interventions and ongoing assessments for clients. You will have an up to date knowledge of mental health assessment and interventions. Experience and knowledge of addictions and current mental health legislation is desirable.
