About the Role
As a Registered Nurse Diabetes, you will provide safe, evidence based, patientcentred care to people living with diabetes across the service.
You will deliver comprehensive assessments, develop and evaluate care plans, and provide education and support to help patients and whānau effectively manage their condition. A key focus of the role is empowering patients through health education, enabling self management and supporting improved long term outcomes.
You will work collaboratively with the multidisciplinary team, including medical, nursing and allied health staff, to ensure coordinated care. The role also involves maintaining accurate documentation, contributing to quality improvement initiatives, supporting students and new staff, and engaging in ongoing professional development.
This is an excellent role for a nurse who is passionate about long term condition management, education and building strong relationships with patients and whānau.

About the Team/Service/ Location
You will be part of the Diabetes and Endocrinology Service at Hawkes Bay Fallen Soldier Memorial Hospital, working within a supportive and collaborative team focused on delivering high quality, patientcentred care.
The service works across hospital and community settings to support people living with diabetes, with a strong focus on equity, education and improving health outcomes for the Hawkes Bay population.

About you
You are an experienced and motivated Registered Nurse with an interest in diabetes care and chronic condition management. You will have:
- Current registration with the Nursing Council of New Zealand and a valid Annual Practising Certificate
- At least 3 years post graduate clinical experience in a medical or diabetes setting
- Ability to work both autonomously and within a multidisciplinary team
- Strong communication, education and relationship building skills
- Good organisational and time management skills
- Competence with documentation and basic computer systems
- A current full NZ driver licence
- Commitment to Te Tiriti o Waitangi and equitable health outcomes
Desired:
- Working towards a postgraduate qualification
- Knowledge of diabetes management and treatment options
- Understanding of chronic condition management and community based care
- A current or developing PDRP portfolio
