Registered Nurse - Community Intensive Pathway at Health New Zealand - Te Whatu Ora Waitaha Canterbury, Christchurch

Are you an experienced and compassionate mental health clinician looking for a role where you can make a real difference in the community?

We are seeking a Registered Nurse to join our Community Intensive Pathway team working alongside a dynamic, multidisciplinary group committed to providing high quality, recovery focused care in the community.

About the role

CIP provides brief and focused care for more acutely unwell consumers in their home while on leave or following discharge from hospital, or to prevent an admission to hospital. The service provides post discharge follow up and increased support to our NGO partners including respite and Te Ao Marama (a community-based peer lead, acute admission alternative service).

We are currently expanding the CIP service, with this role joining three existing positions meaning we can create a dynamic, supportive team delivering increased home-based follow-up in the community. CIP is currently managed by the Clinical Manager of however provides clinical care to consumers under both adult inpatient and community teams.

About you

We are looking for a motivated and skilled mental health nurse who can bring clinical expertise, adaptability, and empathy to this role.

In this role you will:

  • Be able to work independently, undertake home visits on your own if risk assessment determines it safe to do so.
  • Work together in a collaborative manner within a small team, sharing the workload and decision making.
  • Be flexible, confident, experienced and understand the value of intensive community-based support and enjoy working in partnership with consumers and their family-whānau.
  • Have excellent communication skills to effectively work with a range of consumers, family-whānau, colleagues, health providers and other agencies
  • Be self-motivated with drive, passion and initiative

Essential Requirements

  • Registered with the Nursing Council of New Zealand with a current Practising Certificate (APC).
  • Hold a current full driver's licence, valid for use in New Zealand.
  • Experience working in adult mental health
  • Duly Authorised Officer (DAO) or be prepared to train as a DAO and maintain your DAO status while in the role.
  • Knowledge of and a commitment to Te Tiriti o Waitangi
