As a Nurse Practitioner, you will be part of an energetic and friendly Hospital-in-the-Home service, delivering acute care in a community-based setting. This is a unique opportunity to work autonomously while also being supported by SMO collegial guidance, alongside a highly skilled nursing and administration team. The continued expansion of the service, backed by a regional governance platform, makes this an exciting time to join and contribute to innovative models of care.
In your day-to-day role, you will conduct comprehensive physical assessments, diagnose and manage acute and sub-acute conditions, and prescribe medications to support patients at risk of deterioration. You will provide one-on-one care and advice, requiring strong communication, compassion, and proactive clinical decision-making. The role also includes telehealth delivery, staff education and support, and active involvement in service improvement and policy development.
About the Team/Service/ Location
Our Hospital-in-the-Home team provides acute hospital level care to patients in their own homes, supporting better outcomes and patient experiences. You will be working within a collaborative, multidisciplinary environment focused on innovation and excellence in community-based healthcare.

About you
You will bring advanced clinical expertise and a patient-centred approach to care. You will also demonstrate:
- Experience in Acute and Emergency, General Medicine, General Practice, Acute Clinics, or Urgent Care at an advanced practice level
- Ability to work both autonomously and collaboratively within a multidisciplinary team
- Excellent communication and interpersonal skills, with a compassionate and proactive approach to patient care
- Confidence in clinical assessment, diagnosis, treatment, and prescribing
- Interest or experience in telehealth, education, and contributing to service and policy development
